Which investigations are necessary in the patient with erectile dysfunction?

It is very important to distinguish between psychogenic and organic erectile dysfunction. This can be achieved thorough a meticulous examination and history taking. If psychogenic erectile dysfunction is suspected, patients may be further investigated with a nocturnal penile tumescence test (NPT) to confirm that the nocturnal erections are preserved since the presence of normal nocturnal erections excludes the presence of organic impotence. Patients with psychogenic erectile dysfunction should therefore be reassured and usually benefit from counselling.

When the patient presents with organic erectile dysfunction, the underlying cause is often obvious from history taking and examination. When an underlying cause is not obvious from the physical examination and history, the clinician should make an effort to identify possible cardiovascular risk factors, since impotence often represents the first sign of diffuse cardiovascular disease.  

It is in fact paramount to promptly identify the presence of potential cardiovascular risk factors such as diabetes mellitus, high blood pressure and high cholesterol and triglycerides levels in order to set up an adequate treatment plan in collaboration with the patient physician and therefore prevent further progression of the cardiovascular disease.

Among the investigations that should be offered to patients suffering with erectile dysfunction, the Doppler Ultrasound scan of the penis after the induction of an artificial erection provides the clinician with the most useful information regarding the quality of the blood supply to the corpora cavernosa of the penis.
