Penile prosthesis implantation in patients with severe fibrosis of the corpora cavernosa is a very challenging procedure and should be performed only by very experienced large volume surgeons.
Fibrosis of the corpora cavernosa can be caused by a variety of different reasons including previous removal of an infected implant, severe Peyronie's disease, prolonged episode of low flow priapism or trauma.

Dilatation of the scarred corpora can be very difficult, as the soft corporeal tissue has been replaced by hard scar, and therefore the risk of perforation of the tunica and of the urethra can be high. In order to minimize the risk of complication, it is important to have adequate exposure of the corpora performing a second subcoronal incision and degloving the penile shaft to allow for a dilatation under direct vision.
